Service Description

For our patients who are facing dementia, physical health challenges, or are recovering from injury / surgery, this appointment supports patients in their homes under supervision of our FNP.

Cancellation Policy

We do not maintain a cancellation policy. Our practice is that when an appointment is made, we dedicate preparation and research time for our patient's specific needs. If an appointment is missed, we can not fill it and will assess a $40 cancellation fee.
